What is Advance Care Planning
Advance Care Planning (ACP) is a face-to-face service between a me and you to discuss your health care wishes if you become unable to make decisions about your care.
As part of this discussion, you may talk about advance directives (ADs) with or without completing legal forms.
An AD appoints an agent and/or records the person’s wishes about their medical treatment based on their values and preferences.
Benefits
It helps to ensure you receive the care you actually want. It improves ongoing and end-of-life care, along with personal and family satisfaction. Families of people who have undertaken advance care planning have less anxiety, depression, stress and are more satisfied with care. This service is covered by Medicare.
